    The deaths of heart patients in Pakistan have exposed the defects in the health system of Pakistan; following are the main reasons for defective Health system.

    Negligence of Health system by Federal Government and Parliament.

    In past General Zia used to provide moral guidelines to Pakistani nation, once he advised Pakistani nation not to use Ghee as that was dangerous for health of people.
    While present president Zardari never have spoken any single words about such subjects while he himself is suffering from heart disease while Zia himself was not suffering from Heart Disease. Infact Zardari and Parliament removed the subject of health from the federal subjects, while even United nations have not done such thing although that is not a government but maintain this subject in the form of very efficient system of WHO (*World health organization).

    It is obvious from eighteen amendments that present parliamentarians including opposition have made such decisions without thoroughly examining the issue and made blind decisions, therefore Pakistani nation is paying price of their mistakes.All affairs of Health ministry have been transferred from Federation to Provinces, the performance of Provinces in Health sector is obvious from the chronic failure of Sindh province in controlling Polio even in cities like Karachi and Punjab Province performance in controlling **** fever recently.

    At present there are very strict regulations for import of drugs in USA, CANADA EU countries, but in Pakistan due to eighteen amendment Drugs act 1976 have become totally irrelevant. All drugs which are used in heart disease are imported from China and India and no drug is manufactured in Pakistan, Local Pharma Industry just formulate and convert the imported drugs into finished dosage form, and due to ambiguity created by eighteen amendments by foolish parliamentarians, Health system is running without any inspection and monitoring system. Moreover Federal Government have not taken any step for manufacturing of Pharmaceuticals in Pakistan, infact only genuine manufacturing facility of Penicillin Plant in Mianwali established in 1960 have also been destroyed and Pakistani nation is forced to import low quality drugs from Garage based Pharmaceutical Industry in China and India, the quality of their drugs is as follows.

    India
    For Restricting Imports following measures
    Were taken By Indian Government.
    • Ministry of Health and Family Welfare will enforce strict regulatory processes for import of bulk drugs and formulations.
    • For imported formulations, the margin to cover selling and distribution expenses including interest and importer’s profit shall not exceed fifty percent of the landed cost.
    • Due to Government policies and low cost of domestically produced drugs and the absence of patent rights regulations has made the Indian Market less attractive for foreign companies.
    Quality of Indian Medicines.
    “Counterfeit drugs could be the single biggest problem in India in the next ten years due to the growth of garage-based drug manufacturing outfits, rampant corruption, and weak drug control,” A 2003 Scrip report estimated that 15 to 20% of the medicines sold in the country are counterfeits. A case study reported in a 2001 conference showed the following percentage (out of 125 tracer medicines) that failed quality testing: 6% from the public sector, 12.7% from the private sector; and 0% from NGOs.
    In 2001, Lancet reported that, according to WHO statistics, India produces as much as 35% of the fake and substandard drugs in the world. A powerful group of manufacturers have taken over much of the production during the past three years. It is reported that these Counterfeit drugs are manufactured mostly in the northern states; but these fake drugs are widely available throughout the country. They are available as well in Myanmar (Burma) and Cambodia, and their distribution may even extend as far as the former Soviet states, as evidenced by the arrest of four Uzbek women caught trying to smuggle them to be sold in their country. (Ref: 2004 The United States Pharmacopeial Convention, Inc. A Review of Drug Quality in Asia with Focus on Anti-Infective).
    India was among the six countries that participated in a drug quality study which collected a total of 71 samples of the antituberculosis drugs isoniazid (INH) and rifampicin (RMP) as a single entity or a fixed-dose combination (FDC) (see Multi-country studies). Overall, 10% (4/40) of all samples obtained from all six countries, including 13% (4/30) RMP were substandard, containing < 85% of stated content. More FDCs, 21% (5/24), than single drug samples, 13% (2/16), were deemed substandard.

    Raw materials produced in India such as Metoclopramide do not complies to melting point requirement of Pharmacopeia and similarly Iron Protein succinylate produced by Indian companies do not complies to pH dependent solubility.

    China.
    Quality of Chinese Medicines:
    According to the SDA, a nation-wide survey on the quality of medicines carried out in 1998 found that 13.1% of the 20,000 batches tested were either counterfeit or fell below minimal pharmaceutical standards.

    In 1997, China News Digest reported that the Health Ministry of China inspected 1100 medicines and found 138 products that failed to meet national standards. Of these, 48 were fake medicines with pirated registration numbers.
    At the 2002 Global Forum on Pharmaceutical Anti counterfeiting held in Geneva, Switzerland, a representative from the Glaxo SmithKline (GSK) pharmaceutical company reported on the counterfeiting of two of their products in China. The first was Imuran tablets. The counterfeit Imuran was found to contain the correct amount of azathioprine, the active ingredient; however, the tablets were labeled incorrectly as “azathiopring.” Upon testing, the tablets failed the quality specification for disintegration time. The tablets were still intact after four hours in water at 37 °C, while genuine tablets dissolve in 45 minutes. The other drug was Zinacef tablets. The genuine oral dosage form contains cefuroxime axetil; the counterfeit Zinacef tablets revealed the presence of cefuroxime sodium, the injectable dose form. When taken orally, cefuroxime sodium is absorbed minimally by the digestive system resulting in no therapeutic benefit.

    Bangladesh.
    For Protection of Local Manufacturer following measures
    were taken By Government.
    1. The importing of a drug which is the same or one produced in the country, or a close substitute for it, may not be imported, as a measure of protection for the local industry. However, if local production is far short of need, this condition may be relaxed in some cases.
    2. A basic pharmaceutical raw material, which is locally manufactured, will be given protection by disallowing it or its substitute to be imported if sufficient quantity is available in the country.
    3. No foreign brands may be manufactured under license in any factory in Bangladesh if the same or similar products are available/manufactured in Bangladesh.
    4. No multinational company without their own factory in Bangladesh will be allowed to market their products after manufacturing them in another factory in Bangladesh on a toll basis.

    Restrictions on patent rights discourage foreign investors to come up actively in the pharmaceutical market in Bangladesh. Moreover introduction of new molecules is difficult due to slow registration process and restrictions.
    Quality of Medicines from Bangladesh:
    Among the total 245 pharmaceutical manufacturers only top 20 leading manufacturers are producing good quality medicines in the country and most others are engaged in the production of substandard or fake drugs. Substandard or fake versions of life-saving drugs are alarmingly prevalent in Bangladesh markets. In some cases, it is around 70% to 80%.

    One media report showed that among all the pharmaceutical manufacturers only 20 to 25 companies are producing quality medicines in the country. The situation clearly raises a question about the role of the remainder manufacturers.
    They are mainly involved in the production of fake/substandard or imitating renowned brands of various drugs. At present, spurious drugs have been flooded all over Bangladesh. Another testing conducted by the drug regulating body found 69% paracetamol tablets and 80% ampicillin capsules as substandard from some small manufacturers.

    A recent assay involving 15 brands of ciprofloxacin showed that 47% of the collected samples containing active ingredient less than the required specification.
    In a survey conducted during 1988-91, it was found that 66 of the 198 licensed manufacturers were each producing between 1 and 16 substandard drugs (49% OTC drugs and 6.3% injectables, the remainder being non-injectable prescription drugs). Of the OTC products, about 36% was paracetamol and 41% consisted of antacids. The content of the active ingredients was found to be insufficient.
    In 1992, quality studies were conducted on paracetamol tablets, ampicillin capsules, cotrimoxazole tablets/suspensions, vitamin B-complex tablets/capsules/injectables, and vitamin B-2 tablets. A total of 137 brand samples of these drugs were obtained from retail shops in various parts of the country and analyzed for level of content of the active ingredients as well as the disintegration of tablets. Results showed 37 samples were substandard, all manufactured by small companies. Of the 16 brands of paracetamol tablets and 10 brands of ampicillin capsules that were substandard, 11 and 8 respectively, had previously been considered substandard in an assessment conducted by the regulatory authority. This holds true also for the two brands of cotrimoxazole suspension found to be substandard. All analyzed products for 13 of the top 15 companies in Bangladesh met the required standards.

    A recent case study involving a sample of 15 brands of ciprofloxacin collected for chemical assay by HPLC and bioassay revealed seven brands containing active ingredient less than the USP specification.

    Negligence of Role of Pharmacist in Pakistan:
    Role of Pharmacist is very important in USA,CANDA,EU and in other parts of world, but in Pakistan to grasp the whole budget the doctors have made their monopoly in hospitals and do not understand the importance of Pharmacist in the hospital whose main responsibility is to understand the use of drugs,their interactions and side effects, as kingdom of drugs is now so much broad it is not simply possible for physicians to master this field, therefore it is their responsibility and duty to demand and take the professional help from Pharmacist in discharging their duties.

    Deaths due to Heart drugs has exposed the competency of our Doctors and Physicians and their failure of understanding the drugs, These deaths are due to negligence of doctors, as the symptoms which are appearing from the patients using these drugs are the result of having no knowledge of these drugs and their side effects.

    As the most patients are suffering from bleeding and low platelet counts which is a common side effect of one of the heart drug Clopidogrel.

    Side Effects of Clopidogrel:
    This drug main brands in Pakistan are Plavix, Progrel,Rast,Platagg,Loplat,Deplat.
    Most people tolerate clopidogrel (Plavix) well. However, as with any drug, side effects may occur with clopidogrel. When side effects do occur, they may include such things as major bleeding, headache, bruising, flu-like symptoms, and dizziness. Less common side effects of clopidogrel may include stomach bleeding or ulcers, constipation, fatigue, and high blood pressure.

    As with any medicine, there are possible side effects that may occur with clopidogrel bisulfate (sold under the brand namePlavix®); however, not everyone who takesclopidogrel will experience side effects. In fact, most people tolerate clopidogrel well. When side effects do occur, in most cases they are minor, meaning they require no treatment or are easily treated by you or your healthcare provider. In previous research studies, up to 13 percent of people taking clopidogrel stopped their medicine because of side effects.

    Clopidogrel has been studied thoroughly in clinical trials, with over 17,500 people having been evaluated.

    In these studies, side effects are always documented and compared to those that occur in a similar group of people not taking the medicine. The clopidogrel side effects mentioned below occurred in more than 2 percent of people. They also occurred more often in the group taking clopidogrel. These common side effects associated with clopidogrel included:
    • Major bleeding -- up to 3.7 percent of people
    • Flu-like symptoms -- up to 7.5 percent
    • Headache -- up to 7.6 percent
    • Dizziness -- up to 6.2 percent
    • Pain -- up to 5.6 percent
    • Diarrhea -- up to 4.5 percent
    • Upset stomach or indigestion (dyspepsia) -- 5.2 percent
    • Bruising -- 5.3 percent
    • Bloody nose -- 2.9 percent
    • Unexplained rash -- 4.2 percent
    • Itchy skin -- 3.3 percent.

    Because clopidogrel can increase the risk for major or life-threatening bleeding, contact your Pharmacist immediately if you notice that it takes longer than usual for bleeding to stop, you have any unusual bleeding, or you have any of the following symptoms:

    • Bloody nose
    • Black, tarry stools
    • Blood in the urine or stools
    • Vomiting blood
    • Red or purple spots on the skin
    Itchy skin or rash.

    Conclusion:
    From all above analysis it is clear that importing Drugs from India and China is not only harmful for the economy but also dangerous for the health and safety of the nation and measures should be taken to improve growth of Local Pharmaceuticals as well as to promote exports of Pharmaceuticals.

    Laws and regulations about Health should be made by health professional and not by political personalities like Raza Rabani,Ishaq Dar or Zardari or Nawaz Shareef.
    Pharmacists and other health professionals and physicians should work to augment and improve their roles for service of humanity and not just for money making.

  5. Zia used to participate very frequently in Medical Conferences.and First Medical University in Asia and Muslim world "Agha Khan Medical University" was inaugurated in the era of Zia, Only Noble prize recieved to any Pakistani was recieved in his era.
    Cardiovascular Instiute in JPMC was built in his era.
    Medical facilities and education made progress with electric speed in his era.
    While Zardari usually goes for medical check up to Dubai,
    While the Shabaz Shareef even dont know how to treat and control Malaria a thousands years old disease.
    Present rulers are unable to provide medical facilities to Pakistanies which Zia was providing to Afghan refugees in his era, but biased people even then try to hide Zia services to nation.

Why Doctors of PIC are lips sealed about this accident because, they know that they have done a grave mistake which will not only bring Pakistan Pharmaceutical exports to a very low level but will also bring allopathic group of medicines and state and standard of our physicians to a very low level both inside and outside the country.
    At top level Shabaz Shareef is not ready to delegate to any one and in hospitals Doctors are not ready to end their monopoly.

  17. LAHORE: With over hundred dead, Chief Minister Punjab Shahbaz Sharif finally revealed the cause behind the fatal drugs on Wednesday. “IsoTab was found to be adulterated with an anti-malarial.”
    In a press conference held in Lahore, Sharif informed journalists that the drug – IsoTab – produced by Efroze Chemical Industries, was found to have 50mg of an anti-malaria content, which the patients were given every day.
    “The dosage is 14-times greater than the prescribed dosage which is 25mg a week,” Sharif said.
    He said that the Punjab government sent out samples to European labs in order to find out the cause of the deaths. “We not only sent out drug samples to European labs, but also sent human samples, including traces of bone marrow and skin.”
    Sharif said an antidote was also formulated which could reverse the affect of the drug overdose. “Without wasting a second, doctors, paramedics, pharmacists and health secretary of Punjab started to work on the antidote.”
    “Folinic Acid or Calcium Folinade has been found out to be the antidote, which is available in the form of injections,” said Sharif. “We have obtained this drug in large quantities and supplied it to the hospitals. And if necessary, we will even import this drug.”
    He said that experts believed that this antidote will show a positive sign and will save a lot of lives. “Last night, the first dose of this antidote was given to the patients and now, the second dose is underway.”
    The chief minister said that he sent out letters to the concerned officials of the federal government in this regard. “We stayed up all night long to make sure that we get a grip of the situation as soon as possible.”
    “We have told them to seize the spurious drugs and get rid of them.”
    Sharif said that the forensic results were also sent out to the World Health Organisation (WHO) by European labs and that it [WHO] would also do what it could in the situation.
    The chief minister said that the forensic reports were submitted to the Supreme Court and the judicial commission investigating the matter. “There should be no doubts remaining now as we have cleared out everything. The matter should not be politicised.”
    Sharif also called for an “unbiased” investigation in the matter.
    He also informed the media that the factory in Karachi has been sealed by the inspector general of Sindh police.
    On the other hand, Express News’ correspondent Aamir Farooq reported that the company has not been sealed yet. However, a team of Federal Investigation Agency along with officials of health department has reached there.
    FIA Deputy Director Akbar Baloch said that the team will go inside the factory to talk to the management and added that media was not allowed to enter the premises at the moment.
    Farooq reported that there were chances that the company will be sealed after the discussion between FIA’s team and the management of Efroze Chemical Industries is ended.

  20. LAHORE: A laboratory in London has declared Isotab as cause of reaction regarding deaths in PIC.

    A meeting headed by Punjab Chief Minister Shahbaz Sharif was held in Lahore to discuss the issue of spurious medicines. A sample report of the medicines tested in London was presented in the meeting.

    According to the report, Isotab samples were heavily contaminated with Pyrimethamine, which is commonly used as anti-malaria.

    Isotab was manufactured by a Karachi based company Efroz.

    Meanwhile, doctors committee headed by Dr Javed Akram suggested that Folik Acid injections should be used to save patients from further.

    Meanwhile, Chief Minister Punjab Shahbaz Sharif said it is to be probed that the drug reaction matter was a conspiracy or negligence, our sources reported.

    He was addressing the media here today about the spurious medicine issue. 'The pharmaceutical has been sealed and the names of the owners of the company have been put on ECL.'

    To treat the patients affected by the fake drug, an antidote has been produced which is being used in the hospitals, the CM said.

    A laboratory in London declared one of the cardiac drugs as contaminated with an antimalarial, which caused the deaths of more than hundred patients in Lahore.

    According to details, the samples of Isotab (tablet), which were dispatched to a state-of-the-art lab in London, were tested as substandard and injurious to health.

    The lab report revealed Isotab samples to be heavily contaminated with Pyrimethamine, a commonly used antimalarial.

    Traces of antimalarial in the samples of "Isotab" were dangerously high, it added.

    We have to be fair. Zia was no angel but he had some qualities which others (ZAB etc) were lacking.

    He was smarter then Bhuttos, Sharifs ++. Both when it comes to foreign and domestic policies.

    Innocent people are still suffering jails due to laws which he introduced during his islamization .
    Zia-ul-Haq,sent greater than 15,000 rape victims in prison because they couldn't comply with the condition requiring them to have numerous male witnesses of their victimization. Many of those were innocent..

From this accident it is clear that running of Pharmaceuticals by non professional and non pharmacist persons should be discouraged at present MOST OF THE PHARMACEUTICALS OWNERS are businessman such as distributors,and shop keepers, no one can expect care and quality from such peoples,Medicines should not be a business for money making, Therefore Pharmacist should be provided facilities and financial support and loans for running and owning the Pharmaceutical Industries, because only they are educated and qualified for this job.
    Moreover importing Drugs from India and China garage based pharmaceuticals is not only harmful for the economy but also dangerous for the health and safety of the nation and measures should be taken to improve growth of Local Pharmaceuticals as well as to promote exports of Pharmaceuticals.Only when we will hand over Pharmaceuticals business to Pharmacist then only we can compete with India or China, otherwise we will continue to kill our peoples.

    In ninty eighty three, Safia Bibi, a thirteen-year-old blind girl, who claimed rape by her owner and his son was brought to justice for Zina under the Zina Law while, the rapists were acquitted.
    In early nintey eighty eight, another case for stoning to murder of Shahida Parveen and Muhammad Sarwar... both were innocent

Mr.Oblivion it is true that in our country criminals like Veena Malik and Saleem Malik are not penalized, but it is not necessary that a blind girl can not show risky attitude, infact due to being handicapped she can be more inclined for such acts.Infact by protecting criminals we infact support crimes, if few peoples or we are penalized for some crimes, we should encourage them or to ourselves to accept punishment as a sacrifice for maintaining law and order and self discipline in the society instead of weeping for being innocent and for unjustice.
    If discipline and self control can be created in society by even mistaken penalty or punishment to us or to any one, then it is not a bad business but in fact a great sacrifice for society and for our coming generations for prevention of crimes in future.If we can sacrifice our lives for our minor causes then why not a sacrifice for creation of a crime free and self control and law abiding society.

    So what do think, How did zia help in increasing spiritual health of country.     31 Years ago Zia was able to obtain F-16 fighters from USA but after 31 years we are unable even to obtain toy like drones from USA.
    31 Years ago Zia was able to penalize most popular Prime minister in a murder case but after 31 years we are unable even to penalize Ajmal Pahari a street murderer of 100 Pakistani Citizens.
    31 Years ago Zia was termed as a blessing for Pakistan by Vice Chancellor Of Jamma Azhar Egypt but after 31 years our President is termed as rotten head by Saudi King and as a rogue man by Prime Minister of Singapore.

    31 Years ago Zia was able to convinced the world for freedom of Afghanistan but after 31 years we are unable even to convinced the world for respect of authority and respect of borders of Pakistan.

    He hanged Bhutto on signal of American. As Majority of General get order from American. Bhutto in his last era, became a threat to american's plan in subcontinent. So they hanged him using Zia and same history repeated with zia, Zia, in his last era became a threat to american plans and they crashed his plan with their own agent. Do you think i am lying.     didn't Zia aid American in their plan of Russian withdrawal. They used him as long as he remained a puppet.. As he turned against them they crashed his plane. The dream which American were planing 40 years ago, Zia helped them in fulfillment of their dreams. Do you not see American around (Afghanistan). Tell me how did he finished American influence upon Pakistan goverment. Pakistan goverments, after zia, still remained as American puppet.

At present ministry of health in Sindh specially Drug Testing Laboratory are being handled by sister of Mr.Asif Ali Zardari, Uzra Peechu, her son Shahid Peechu who is also nephew of Zardrai has been appointed as a Deputy Drug controller, in Sindh all competent drug Inspectors such as Najmus Saqib, Muhammad Amir,and Aslam Jan Khattak have been transfered on the first day of this corrupt government and replaced by Zardari family cronies like Bouzdar,Suresh Kumar,Tanveer Alam, these are recoganized corrupted officers and have been penalized by court of laws.Their job is just collection of bribes and money from Pharmaceuticals.That is the reason that Pharmaceutical Industry is now very relaxed in Karachi,because they just pay money and no one ask any thing about Good manufacturing practices or safety of drugs being manufactured by them, monthlies of Drug Testing Laboratories and Drug Inspectors are fixed.In such scenario one can only expect and forcast disaster and nothing else.
